Spoken commander name is using abbreviation even when converted to phonetic

Expected

My commander name pronounced correctly (Doctor Evil)

Observed

No matter what I try, "Doctor" is pronounced "DR".
I am using the phonetic ˈdɑktər ˈivəl

I don't see any information here about the voice you are using?

If you're using an Ivona voice, there are some issues with IPA pronunciations that we have never been able to fully surmount, described here: #297

You may be able to use a lexicon (see https://github.com/EDCD/EDDI/wiki/Lexicons) as an alternate method to affect the pronunciation.
