LGTM from code-reading, but note that 1.91.0 is broken for some macOS users. This should be resolved in 1.91.1.

Would it make sense, for simplicity, to just wait for 1.91.1 to be released? I don't think there's any rush... |

This PR adopts a new VS Code API that should improve the UX around debugging unit tests.
