Problem/Bug with FBIS and Contact option

There is a 'server error' when reporting problems on the FBIS site using the 'Contact us' option in the ribbon.

My query is about adding a new site. When adding a wetland, there is no response when clicking on "Fetch wetland name" and "Fetch data". Only "Generate site code" works. This prevents me from adding a wetland site, even when the checkbox is clicked ("I agree to these data being shared via the FBIS platform for visualisation and download by registered FBIS users").
