Percentage median BMI implementation #75

Percentage median BMI is used widely in child and adolescent eating disorders, and may have some utility in obesity, though SDS in obesity seems to be a more commonly used measure.

The RCPCHGrowth package reports %mBMI for both corrected and chronological ages, so should be included, perhaps in the tool tips against BMI.
