Fields not properly being populated when editing a search #170

The Clinician Name(s) and Service Type fields are not being properly populated when editing a search.
Steps to reproduce,

Submit a search w/ multiple clinicians and services
Click submit
Click back. You should see only one name and one service type populated in the fields.

The component is not properly populating the fields using the search data obtained from the search API
